Endometriosis involves the presence of endometrial tissue outside the uterine cavity, often on organs such as the ovaries and behind the uterus. Similar to the endometrium in the uterine cavity, this misplaced tissue reacts to hormonal changes, resulting in symptoms like painful periods, infertility, and pain during intercourse. While no definitive blood tests confirm endometriosis, the presence of blood-filled cysts in the ovaries on pelvic ultrasound strongly suggests endometriosis in infertile women. Laparoscopy remains the gold standard for confirming the diagnosis.
The approach to endometriosis treatment depends on factors such as the extent of the disease, symptoms, and cyst size. In addressing infertility, the focus is on achieving pregnancy. For women with moderate to severe endometriosis, the treatment typically involves a combination of laparoscopic surgery followed by medication to suppress the disease. Unfortunately, there is no complete and permanent cure for endometriosis, and both medication and surgery provide temporary relief.
Laparoscopy is an effective tool for diagnosing and correcting various forms of endometriosis. In mild cases, cauterization of endometriotic areas on the ovaries' undersurface and uterosacral ligament may be performed. For ovarian endometriotic cysts (chocolate cysts), laparoscopic procedures involve drainage, cauterization of the inner cyst surface, and peeling out the cyst wall. Adhesions between the ovaries and their base can be freed through laparoscopy in severe cases. Additionally, laparoscopy can address adhesions involving the uterus and fallopian tubes.
Endometriosis is a recurring condition, and patients may experience a return of endometriotic cysts, necessitating further surgical intervention. Complete surgical removal can keep patients symptom-free for up to 5 to 6 years. Deep infiltrating nodules may require excision, and bowel endometriosis, if present, is surgically treated.
